How do I change the categories and email addresses on the contact form?

To change the categories and email addresses on the contact form, go to Administration > Site settings > Manage contacts.

How can I add media inquiry information, such as phone numbers, addresses, etc. to the Contact form?

To add media inquiry information such as contact names, phone numbers, address on the Contact form page, create a custom block with this information and have it display on the Contact form page. For steps on how to add a custom block, see the user guide.
